Ban appeal help

Hello all,

I haven't been on this server in years (I can't remember how many, but I recently started playing a little minecraft recently, and I found that my account was banned here. When I try to connect, it tells me I am permanently banned for fly hacking. I want to say that this was years ago and I wouldn't ever do something like that now. However, I understand that this isn't the place for appealing my ban so I'll leave details for later.

I went to the ban appeal section of the forums, but I couldn't find a way to post an appeal. Does anyone know how I can submit an appeal?
